The dissertation is devoted to study of the role of vascular endothelium dysfunction in development and progression of heart failure in patients with Ischemic Heart Disease in combination with Hypertonic Disease and possibility of its correction by ACE inhibitors (enalapril) and b-adrenoblockers (carvedilol). 203 patients were studied and in 162 of them (79,8%) Chronic Heart Failure, functional class I-III was diagnosed.
Increase of the incidence and aggravation of endothelium dysfunction were shown to rise as HF progressed. Endothelial dysfunction was associated with index of left ventricular myocardial mass, the size of myocardial stress, change of the ratio end-diastolic exchange/myocardial mass of the left ventricle, with parameters of systolic and diastolic function of the left ventricle as well as with disturbances of lipoperoxidation processes. In patients with Chronic Heart Failure and evident endothelial dysfunction the influence of sympaticus was clearly marked. Combined use of enalapril and carvedilol showed advantages of their influence on the parameters studied.
